Warranties

Our funeral home makes no representation or warranties about the protective value of certain caskets or outer burial containers
other than those made by the manufacturer. The only warranties, expressed or implied, granted in connection with goods sold
with this funeral service are the written warranties, if any, extended by the manufacturers thereof. No other warranties and
specifically, no warranties of merchantability and fitness for a particular purpose are extended by the seller.

Financial Policy

Payment in full is required 72 hours (3 days) prior to furnishing of any merchandise, ceremonies or disposition. We accept cash,
cashier’s check, VISA, MasterCard, American Express and incontestable/assignable life insurance policies.

There will be a 7% processing fee on all life insurance policies added to the Cash Advance Section of your Statement of Funeral
Goods and Services selected. Any additional cash advance items selected after the Statement of Funeral Goods and Services
have been prepared and signed, will be billed to the contracting party, and payment is expected within 30 days. Any outstanding
invoices will incur an 18% charge each month it goes unpaid. California Sales Tax (9%) will be added to the cost of
merchandise purchased.

All information given to the caretaker at the time funeral arrangements are made must be correct vital information prior to filing
with the State of California. It is the contracting party’s responsibility to provide the counselor with the correct information. If the
information given is incorrect after the filing process has taken place, the death certificate must be amended. This can take
several weeks and you will be responsible for all additional charges incurred to amend and order corrected copies of the death
certificate.

Burial and Cremation Packages

We have grouped valuable services and merchandise into convenient packages that will simplify the funeral planning process and
offer significant savings over the cost of individually itemized services. We have discounted the price of the items included in the
packages (exclusive of the basic services fee) to provide an overall savings over the prices of the same set of goods and services if
selected on an itemized basis. Packages are only available to families who purchase the casket from Olive Tree Mortuary, Inc.

Our packages include only the items listed in each individual package. Additional merchandise you select and the cash advance
items are not included. The non-use of the goods and services contained within any of the package selections will not result in a
reduction in the price of the package selected.
